Dear Parents/Guardians,

The state of Maine now requires that schools obtain positive permission (a form that is signed by you the parent/guardian) for your child to use the Internet and e-mail in a supervised setting. We would like all students to participate in these activities on the computer. We would also like to have your permission to include information about your child on our web pages. Please sign this permission slip and have your child return it to school by .
